As of Monday 11th July 2022, Sport Canterbury will no longer accept Green Prescription self-referrals for Ashburton and Greater Christchurch due to the conclusion of our contract with the Canterbury District Health Board.

Clients residing in South Canterbury, West Coast or receiving support through the Esculated Care Pathway or Diabetes Education will continue to be able to self-refer to Sport Canterbury using the form below.
